Personal data refers to any information about an identified or identifiable natural person; an identifiable natural person is one who can be directly or indirectly identified, especially by reference to an identifier such as a name, identification number, location data, online identifier, or one or more factors specific to the physical, physiological, genetic, mental, economic, cultural, or social identity of that person.
The Controller has not appointed a Data Protection Officer.
II. Sources and Categories of Processed Personal Data
The Controller processes the personal data that you have provided via the contact form on the website:
- Name and surname
- Phone number
- Email address
- Optional note
- Preferred date for an appointment
The Controller processes your identification and contact details solely for the purposes outlined in Article III.
III. Legal Basis and Purpose of Personal Data Processing
The legal basis for processing personal data is your consent for processing your data for the purpose of responding to your inquiry submitted via the contact form, pursuant to Article 6, paragraph 1, point (a) of the GDPR. The purpose of processing personal data is to handle your inquiry submitted via the contact form and to fulfill related rights and obligations. The Controller does not engage in automated individual decision-making within the meaning of Article 22 of the GDPR.
IV. Retention Period for Personal Data
The Controller will store your personal data for the period necessary to respond to your inquiry and handle related claims (up to 2 years from the resolution of the inquiry). Once the retention period has expired, the Controller will delete your personal data.
V. Recipients of Personal Data (Controller’s Subcontractors)
Recipients of personal data include: Persons involved in handling your inquiry. The Controller does not intend to transfer personal data to a third country (outside the EU) or to an international organization.

VII. Conditions for Securing Personal Data
The Controller declares that it has taken all appropriate technical and organizational measures to secure personal data. The Controller has implemented technical measures to secure data storage and physical storage of personal data, including password protection and antivirus protection. The Controller declares that access to personal data is granted only to authorized persons.
VIII. Final Provisions
By submitting the contact form on the website, you confirm that you are familiar with the Privacy Policy and fully accept its terms. You agree to this Privacy Policy by submitting the contact form, which explicitly and clearly states that by submitting it, you consent to these terms. The Controller reserves the right to modify this Privacy Policy. The updated version will be published on the Controller’s website and will also be sent to your email address provided to the Controller.
These conditions take effect on August 1, 2024.
